I understand that your time and access to equipment can be limited, and that not every owner is able to travel out of the county to see a diplomat. I also understand that ophthalmology cases can be daunting!

Refer a Case

I offer a comprehensive ophthalmology referral service for the diagnosis and management of medical or surgical ophthalmic issues. A report will be sent to you in the days following the appointment to keep you up to speed (but please poke me if you need an update sooner!)

All first referrals have to be approved once requested - please ensure that clients understand this.

Please input the client details for "your" details in the form. There is a space for you to input your practice details.

There is a 3 hour advanced booking limit set for this - once again, if you have something you feel needs seeing sooner that day please let me know.

For first referrals the history still needs uploading (through the booking system) or sending separately. You are also welcome to fill in a referral form online and we'll contact the client directly - this system is simply another option for those that may prefer it.
